Completing this form before booking ensures that patients who are unlikely to qualify are not charged for unnecessary consultations.
To proceed, you’ll need to supply your Summary of Care Record or relevant medical records from your GP or specialist.
This information allows our clinicians to confirm your diagnosis, review your treatment history, and ensure medical cannabis is a safe and appropriate option.
If you’re unsure how to obtain these records, our team can guide you through the process. Most GP practices can issue a Summary of Care Record upon request, free of charge.
Once your eligibility is confirmed, you’ll be invited to a video consultation with one of Auravia’s specialist consultants.
During this appointment, you’ll discuss your condition, medical history, and treatment goals. Your consultant will explain potential product types, dosing methods, expected effects, and safety considerations.
If medical cannabis is suitable, your specialist will outline a treatment plan tailored to your needs.
After your consultation, Auravia’s clinical team will complete a case assessment to confirm your suitability for treatment.
This process usually takes 2–3 working days and will never exceed a week.

Once approved, your prescription will be sent directly to our partner pharmacy, Petal & Bud. You’ll be able to pay for your medication securely through their online system.
By UK law, medical cannabis prescriptions are valid for one month’s supply only. This means that even after your treatment is established, you’ll need to check in monthly to request and pay for your next prescription.
